Empowering Change: Understanding and Applying the Transition-To Success (TTS) Model

This workshop provides participants with a comprehensive introduction to the Transition to Success (TTS) model—a framework that treats poverty as a condition, not a character flaw. Grounded in the Diagnosis Poverty methodology, the session helps attendees understand how TTS integrates social determinants of health, goal‑setting tools, and coaching practices to drive sustainable personal and organizational change.

Throughout the workshop, participants explore TTS’s core components, including foundational tools such as the Life Area Survey, Map of My Dreams, Coach Curriculum, and Fidelity Checklist. They learn how certification pathways support high‑quality implementation and how organizations can embed TTS into workflows, treatment planning, and evaluation processes.

The session is highly interactive, inviting participants to design a mock implementation plan, practice using TTS tools, and develop evaluation and data‑collection approaches. By the end, attendees leave with concrete next steps—such as identifying actions to advance TTS within their own agencies—and a clearer understanding of how TTS can foster empowerment, improve outcomes, and strengthen organizational alignment.
